· Arriving on-time for your appointment
· Excellent oral hygiene
· Cooperation with wearing rubber bands and appliances
Bonus Points Will Be Given For:
· Referring a friend to our practice
· Visiting our website (limit 1 point)
· Completing community service hours (verification necessary)
· Good report card grades (B average or better; report card must be present)
Points Will NOT Be Given:
· At an appointment to repair lost or broken appliances
· If the patient missed their last appointment without calling in advance to cancel
